The Licensed Practical Nurse (LPN) is responsible and accountable for the delivery of nursing care to veterans at the Lexington VA Outpatient Clinic located in Somerset, Kentucky. The LPN is an essential component of the PACT Initiative (Patient Aligned Care Team). The incumbent has the knowledge and skills necessary for concentrated efforts and self-direction in carrying out the most complex practical nursing care assignment with patients ranging in age from young adulthood to the senior. The LPN (PACT): Possesses the knowledge and ability to carry out patient care duties, including assignments in selected inpatient and/or outpatient care settings. Provides float coverage for PACT LPN staff relief during times of vacancy, scheduled, or unscheduled absences. Provides care to veterans requiring complex practical nursing care. Demonstrates sufficient skill to resolve a range of nursing problems. Displays a professional appearance and attitude while interacting with patients, visitors, and other VAMC personnel. Operates with technical soundness, appropriateness, and conformity within policy and procedures. Possesses the ability to observe, identify, and respond to patient's needs for care including medication, equipment-assisted care, and patient/family education. Possesses the knowledge and ability to recognize urgent or emergent patient care situations, notify the Registered Nurse and/or preceptor and/or physician provider, and initiate appropriate emergency interventions. Possesses knowledge and understanding of human behavior, patient motivations/reactions to situations, and the ability to appropriately utilize this knowledge in working effectively with patients, family members, and other staff. Actively seeks out educational opportunities to enhance nursing knowledge and skills, sharing new knowledge gained with other staff to improve and advance nursing practice.
