The North Florida/South Georgia VA Healthcare System is recruiting for two fulltime Advanced Practice Registered Nurses to work within our Mental Health Service at the Jacksonville, VA Outpatient Clinic. The VA Jacksonville OPC provides a broad range of general and specialized medical, dental, surgical, psychiatric, Nursing and Ancillary services. Come join our team of health care professionals in caring for our Veterans The Advanced Practice Registered Nurse (APRN) performs a variety of services using advanced practice nursing process, competency and skills to perform the following: Assess, diagnose, monitor, promote health and protection from disease, and manages various actual or potential conditions, illnesses, disorders, and injuries of patients presenting to the Psychiatry-Primary Care Mental Health Integration (PCMHI) Clinics/Programs Assess, stabilize, reduce risk, and determine disposition of patients with various urgent conditions, illnesses, disorders, and injuries consistent with medical staff policy regarding emergency and consultative call services Using established processes admit PCMHI clinics/program patients to the hospital, order VA specialty consults, and initiate requests for non-VA care Performs the documented history and physical examinations and develops and order plans of care inclusive of behavior modification modalities when applicable and using holistic models of care approach Orders and/or performs appropriate PCMHI clinics/programs focused and related laboratory, imaging, and diagnostic tests, and integrate the results into clinical decision making Provides PCMHI clinics/programs focused and related counseling and education to patients, families, and caregivers Triages PCMHI clinics/programs consultations, and initiate/expedite added PCMHI clinics/programs consultation for patients with PCMHI related conditions, illnesses, disorders and injuries Manages acute and chronic PCMHI related conditions, with focus on health promotion, protection, and preservation, disease prevention, risk reduction, behavior modification, and patient education and counseling Conducts preparatory work-up for PCMHI clinics/programs procedures and post-PCMHI clinics/programs procedures management Prescribes, orders and administers formulary drugs in accordance with VA regulations, guidelines of federal Controlled Substances Act, and state of licensure laws and regulations Obtains informed consents for the PCMHI clinics/programs focused or related minor and major procedures and interventions Performs PCMHI clinics/programs focused or related routine tests and procedures as determined by the PCMHI Clinics/Programs specialty/subspecialty requirements, practitioner education, training and experience, and clinical needs of the patient, using evidence-based practice, clinical guidelines, policy, procedure and protocols as applicable Conducts individual, group, and family therapy, performs crisis intervention services, emergency psychiatric evaluations, and initiate/implement involuntary inpatient hospitalization procedures; order consults and placements for continued mental health (MH) and substance use disorders (SUD) specialty residential care; order consults and assessments for participation in MH and SUD programs Work Schedule: Monday-Friday 8:00 - 4:30, may include Saturdays Financial Discloser Report: Not required
