The position is assigned to Behavioral Health Service at the Mann-Grandstaff VA Medical Center in Spokane, Washington. The LPN is responsible for observing, recording and reporting changes in the patient's condition, as well as assisting in the development and revision of the patient centered care across the mental health continuum of care The LPN demonstrates active collaboration and communication within the Behavioral Health Interdisciplinary Program (BHIP) Teams and other BHS staff as appropriate. These disciplines include Psychiatry, Psychology, Social Work, Nursing, Licensed Independent Mental Health Counselors, Peer Support, MSA's and administrative support staff. The LPN must have the ability to react to situations in accordance with established policies and procedures, and ability to appropriately use this knowledge to work effectively with a wide range of patients. 1. Makes observations and thoroughly documents the patient's care and condition. Appropriately seeks assistance and guidance by informing the Provider or RN of any condition requiring higher-level intervention. 2. The LPN performs, reports and records assigned nursing care in Behavioral Health Service, including, but not limited to: a. Medication Administration b. Clinical recommendations completion with appropriate consults and alerts, as indicated. c. Check expiration dates of medications and supplies d. Delegated procedures and treatments e. Timely documentation in electronic medical record f. Performance of EKG's 3. Adheres to standards of practice and safe techniques in preparation and administration of medication. 4. Demonstrates understanding of age-related and population-specific characteristics that affect the health care needs of the Veteran. 5. Promptly recognizes urgent or emergent patient care situations, seeks 2 assistance and effectively intervenes as directed. 6. Provides patient and family health education, to include self-management, prevention and wellness, utilizing the patient's stated goals. 7. Interacts and communicates appropriately with patients and family members. 8. Demonstrates professional behavior (respect, civility, ability to accept constructive criticism), effective communication, teamwork and interpersonal skills. 9. Adheres to the principles of infection control, safety, and universal precautions. 10. Possess knowledge of proper handling and pre-cleaning of reusable medical equipment. 11. Demonstrates initiative in assisting the various specialties within BHS. 12. Follows policy and procedures related to fire and disaster drills, safety and security measures. 13. Demonstrates competence utilizing special equipment needed to perform job duties. 14. Assists in the orientation and competency validation of students, peers and clerical staff. 15. Maintains confidentiality of electronic, written and/or verbal patient information. 16. Maintains a work environment that honors diversity. 17. Serves as a resource and advocate in the protection of patient and family rights. 18. Completes mandatory reviews and training, demonstrates competency in unitspecific procedures. 19. Supports patient-driven care in a safe, efficient and cost-effective manner, using evidence-based principles. 20. Promotes patient and personnel safety. 21. Actively seeks out educational opportunities to maintain and enhance nursing 3 knowledge and skills. 22. Perform communications and facilitate safe transitions of patients between BHS and other healthcare settings using informal and formal communication methods, as appropriate. 23.Technically proficient in initiating, performing and completing care of the Veteran population. 24. Recognizes and reports unsafe equipment, conditions, and practice. 25. Meets or exceeds all standards related to critical elements of the position, including Personal Mastery, Customer Service, and Technical Skills.
