Under general supervision and according to established policies and procedures, establishes a partnership with patients/families and hospital personnel to enhance a shared approach to the delivery of health care. The job duties include: 1. Greets patients and families upon arrival to the patient care unit and orients them to the Patient and Family Centered Care Program. 2. Documents the identified Care Partner. 3. Answers questions from patients and their families, and explains procedures and processes within the scope of their training/abilities. 4. Conducts patient rounding. 5. Maintains availability to meet with families daily. 6. Communicates with appropriate staff regarding patient/family issues and/or concerns. 7. Assists the health care providers with servicing patient/family requests. 8. Collaborates with nursing staff to accommodate priority visits. 9. Assists with answering phones and call lights at the nursing station. 10. Acts as Liaison between clinical staff and families in critical or emergent situations. 11. Documents and conducts initial investigation of complaints of a minor/serious nature. 12. Completes correspondence and mailings as requested by management. 13. Monitors visitor reception areas for cleanliness and maintains amenities in reception areas. 14. Compiles unit specific data as requested by management. 15. Enhances personal growth through participation in workshops, educational programs, in-services, and reviewing current literature. 16. 1. Associate?s degree in Healthcare Management, Business Administration or related field, or the equivalent combination of education and/or experience. Bachelor?s degree preferred. 2. Previous customer service experience in a health care environment strongly preferred. [BRASSRING IMPORT 11/4/16]
